Portraiture for mild stuff, heavy PS or outsourcing for more.

Depends on the editing needed.
I dont like the Portraiture Plug-In for LR, so I use the PS version...so that means, pretty much anytime i shoot portraits, im in PS for at least a few seconds doing some of my editing(running the Portraiture plug-in which is FANTASTIC by the way...). If there is a REALLY bad blemish, ill roughly clone it out and let Portraiture take care of the rest of the smoothing, blending.
For mild tweaks like color, contrast, exposure, vignetting, sharpening, etc, i use LR. When not shooting portraits, i rarely use PS unless i need to do heavy cloning but for portraits, PS is a necessity for me.
They need to hurry the hell up with LR3 already!!
